What is a SOAP Note?
Our template follows a common medical structure, which includes Subjective, Objective, Assessment, and Plan. This type of note has long been used in the medical field but is also suitable for other industries and tasks.
Simple and Eye-Pleasing Design
The layout consists of a header section and four separate blocks for your notes referring to the SOAP structure. In the header, you can specify the name, date, and diagnosis (or replace the placeholder text for other industries).
